Who qualifies
Age 55+, certified as needing nursing-home level of care, but able to live safely at home with PACE support. Must live in a PACE service area.
What you get
One program covers ALL your medical, dental, prescription, day-program, transportation, meals, and home-care needs. Replaces Medicare and Medicaid as your delivery system.
Tips that matter
